The Goal
As we age, a goal we all have is to remain at home as long as we can. Unfortunately, ERs, hospitals, and nursing homes are often the reality. Not only does remaining at home improve the quality of a person’s life, but financially it can save thousands of dollars. The Revolutionary Health Services Home Care Program provides a way to make this goal possible, physician home visits.
How it Works
Keeping people at home sometimes requires a coordinated effort and optimal communication among the family, physician, and any necessary caregivers. With the RHS Home Care Program, this seemingly impossible task is routine and you can trust that everything possible is being done to make their daily life enjoyable.
The plan includes:
- Quarterly routine home visits- Maximizes the management of medical problems to identify developing problems early.
- 10 non-routine home visits yearly for problems that arise.
- Weekly care plan consult between the physician and a designated caregiver.
- Close coordination of care and medications from specialists.
- Cell phone and email access to physician for family and caregivers.

- Home visits- Eliminates a senior’s inability/unwillingness to go to the doctor’s office
- Avoiding ERs- If problems are addressed early in the process, ER visits can often be avoided. A few hours sometimes makes the difference.
- Avoiding Hospitals- Some medical problems that are typically cared for in the hospital can be treated at home with the proper care team.
- Avoid nursing homes- A possibility when an adequate care team can go to the home.
Cost of the Program
Individual $3,900 yearly- the cost of 16 days in the average nursing home*
Couple $5,700 yearly

Who should take advantage of this program?
Answer:
Families who are even beginning to discuss the possible need of a nursing home for a loved one who is having difficulties at home.
Anyone whose medical condition makes physician home visits preferable.
What if additional non-routine visits are needed?
Answer: More are available at an additional fee.
Do unused non-routine visits roll over to the next year?
Answer: Yes
What if the patient dies?
Answer: The unused balance paid is credited to another family member or refunded. Banked non-urgent visits are not refunded.
Will the fee increase?
Answer: The fee will rise once yearly in accordance with the Consumer Price Index (typically <4%).
